Our Case Management Services team brings extensive experience and expertise to the individuals and families with whom they work. We value each member of the child’s team, from educators and health care professionals to family members and caregivers. Our goal is to create strong relationships with all the players to benefit the child’s healthy development.Our services are tailored to each individual’s needs and circumstances, and might include assistance with:
- Working with families through the process of applying for and securing MaineCare, Katie Beckett, Social Security and other financial assistance - Experience and high success rates in helping families secure funding
- Establishing, coordinating and monitoring in-home supports and healthcare services
- Helping families secure respite and flex funding and finding appropriate respite and recreational opportunities
- Supporting the transition from school to adulthood
- Supporting IEP development
- Vocational, crisis and residential planning
To qualify for Pine Tree Society’s Case Management Services, children must:
- Be age six to 18
- Have a diagnosis of Mental Retardation, Pervasive Developmental Disabilities (PDD) and Autism Spectrum Disorders
- Reside in York, Cumberland, Oxford, Sagadahoc or Androscoggin counties
