Adult Case Management

Pine Tree Society provides high-quality targeted case management services to meet the needs of Maine adults who are eligible for services under the guidelines of the Department of Health and Human Services.

Our goal is to assist in the development, coordination and facilitation of a comprehensive person centered plan designed to meet the specific needs of the people we serve.  Our team will emphasize community inclusion and empower the individual and their family to be as independent as possible while ensuring that the receive the support they need.  We are committed to facilitating a supportive relationship between individuals, their families and service providers in the continuum of care.

Adult case management assists in the coordination of vocational, residential, recreational, FSD programs, medical and dental care and independent living skills.
